Registration instructions:

(we need you to submit here electronically and as well as have a hard copy with your signature.)

Before submitting or printing;

1.)Change the information in each text box in the gray area of the form.

(This is required in order to have the smite button to work properly.)

2.)Print the page before you hit the Submit button,

(This is so you will have all the information printed nice and neat.)

3.)Sign and date the completed and printed form at the bottom, then place in a envelope along with any fees,

(If you wish to have your pet photos returned to you, please make sure you fold in a return envelope with proper postage fee.)

4.)Send by US post to:

(please; for your protection; if your sending fees bye way of us postal, use check or money order, if you have cash, then step number 5.)

5.)Plan a trip to personally deliver your entry to the clerk at Sky Lane Bowling Alley or the Middlefield Bank

(Garrettsville offices only)

6.) OR Simply bring the envelope along with any fees to the event.
